Can you give an example?

Assume we are trying to improve the performance of the Pizza organisation, which has numerous outlets. Apparently at random, different outlets have periods where they fail to deliver pizzas within forty minutes at peak periods, leading to an increasing number of dissatisfied customers. 

The critical success factor we are considering is Delivery Time and the critical failure level has been assessed at 40 minutes.
